Position located in Middletown CT

VEVRAA Federal Contractor: Priority Referrals of Protected Veterans Requested


To provide outreach and engagement services for individuals and households who are experiencing homelessness due to mental health and addiction in the Greater Middletown area. Providing connections to community resources and basic needs; ultimately assisting in improving their quality of life.

Qualifications, Knowledge, Skills and Abilities
Bachelor’s degree with 3 years relevant work experience required. Driver’s license with acceptable driving record and competent writing skills. Knowledge of community support systems and resources, counseling, interpersonal skills, ability to understand and carry out oral and written instructions, ability to evaluate and act in crisis situations, knowledge of leasing procedures and fair housing laws, knowledge of housing quality standards and inspection methods. Time management skills and computer literate with a deadline oriented mindset. Able to walk in the community outreaching potential clients. Able to meet all documentation requirements. Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics; capacity to work within a team and foster teamwork. Crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, motivational interviewing, person-centered-planning and strong empathetic skills all highly desirable. Ability to maintain her or his own wellness. Proficiency with MS Outlook, Excel and Word.

Duties and Responsibilities
Improve the client’s quality of life by, among other things: providing linkages for those who are in need of social, mental health, educational, vocational, recreational, spiritual, HIV, and/or substance abuse services. Provide transportation to appointments as appropriate; coordinate and collaborate with other agencies. Attend weekly and monthly agency, program and network meetings. Monitor all cases assigned and complete all documentation and charting requirements. Other related duties as assigned by supervisor.

Salary Range: $16.75-18.00 per hour plus paid time off
Schedule: Monday through Friday, 8:00 am – 12:00 pm
