Middlesex Scattered Sites Case Manager

Purpose

The Middlesex Scattered Sites Case Manager seeks to provide case management services to formerly homeless adults, many HIV positive, now living in long term supportive housing apartments in the Greater Middletown area, in order to assist individuals in maintaining housing and improving their quality of life.

Duties and Responsibilities
Meet with clients regularly to provide person-centered services and case management interventions. Engage and empower clients to improve their quality of life while documenting service plans and progress. Case Manager will employ evidence based approaches within program limits, including; housing first, motivational interviewing and harm reduction. Assistance in providing clients linkages to clients who are in need of social, mental health, educational, vocational, recreational, spiritual, and medical and substance use services. Case Manager will collaborate with community providers and landlords to develop a strong network of support for clients being served. Timely and accurately enters notes and data into Homeless Management Information System (HMIS) which generate requested funder reports. Provide client transportation and some ADL skill building. Attend required CHI, program and network meetings. Monitor cases and complete all required documentation/charting. Maintain strict confidentiality. Assist with special projects and complete other related duties as assigned.

Qualifications
Master's Degree preferred or Bachelor's Degree with 2 years of related experience. Valid Driver's License. Able to effectively use the local network of substance abuse, mental health, and social services providers. Familiar working with homeless populations is a must and persons with diagnosis of HIV is a plus. Time management skills and computer literate with a deadline oriented mindset. Able to meet all documentation requirements; complete knowledge base of all aspects of the delivery of person-centered case management. Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics; capacity to work within a team and foster teamwork. Excellent writing skills with the ability to complete assessments is required.

Supervised By: Program Manager of Middlesex Services
Salary Range: $17.23-$19.23/hr. based on experience plus excellent benefits
Schedule: Monday through Friday 9:00 am to 5:00 pm
