Purpose and Functions
This position will use the SSI/SSDI Outreach, Access and Recovery (SOAR) Model with DMHAS Forensic populations to assist in accessing mainstream benefits. Eligible participants will be transitioning into the community, living in transitional programs or at risk of losing housing.
Duties and Responsibilities
Coordinate the SOAR application submission process for Forensic populations in the Middlesex and Hartford regions through a comprehensive review of each application for accuracy and completeness. Provide technical assistance regarding electronic submissions to the Social Security Administration and entering of submission data into federal database; be liaison to Medical Records and other hospital departments, Disability Determination Services, Social Security district office and other relevant services when necessary to secure provision of documents; direct the appeals process as necessary. This position will also participate in planning meeting(s) to establish collaboration among key state and/or local; stakeholders continuing to implement a SOAR approach (e.g. SSA and Disability Determination Services (DDS); State Mental Health Agency and Department of Corrections leadership; and community homeless, health and behavioral health providers). Responsible for monitoring and reporting on activities to DMHAS and key stakeholders. Other related duties as assigned by supervisor.
Bachelor's degree in human services related field; working knowledge of behavioral health service models including the recovery model; and trauma informed service delivery; strong familiarity with Social Security Administration processes for applying for federal benefits. Minimum 3 years of experience in a professional work environment with proven ability to communicate both verbally and in writing; the ability to collaborate successfully with others; demonstrated time-management and organizational skills; and the ability to meet deadlines and focus on results; Advanced understanding and proficiency with Microsoft Office Suite including MS Outlook, Access, Excel and Word. Driver's license with acceptable driving history required.

Salary: $17.78 to $19.23 per hour based upon education and experience plus excellent benefits
Schedule: Monday thru Friday 8:00 a.m. to 4:00 p.m. with some flexibility
