Supportive Services for Veteran Families (SSVF) Case Manager
Location: New London

Supportive Services for Veteran Families (SSVF) Program provides supportive services to very low-income Veteran families in or transitioning to permanent housing. Through the SSVF Program the lives of very low-income Veteran families’ improve through housing stability. This program will provide eligible Veteran families with outreach, case management, and assistance in obtaining VA and other benefits. These benefits may include employment assistance, behavioral health care, legal services and financial assistance.
Individual must be able to work both independently and within a team setting. A strong candidate is highly organized and attentive to detail. Motivated to complete tasks and see consumers succeed is also important. Excellent writing skills and a valid driver’s license with a good driving record a must; computer literate with the ability to quickly learn the HMIS database used by the SSVF program; must be able and willing to accept direction and guidance from supervisors. Bi-lingual English/Spanish preferred. Bachelor’s degree required or Associates degree with 2 years related work experience.
Duties and Responsibilities
Assist and monitor SSVF housing consumers from entry into the SSVF program until successfully discharged. Outreach and engage clients as necessary; provide intensive site and mobile case management services. Identify housing assistance options and assist with securing affordable housing within 90 days of program entry, ongoing check-in with consumers to meet their housing needs. Assist with maintaining or creating connections with mental health, substance abuse and other health care providers. Maintain consumer records in a manner consistent with project and agency standards including both paper and electronic records. Manage a consumer case load of approximately 30 individuals at any one time and other related duties as assigned. Must have reliable transportation to attend trainings, meetings, events and carry out all assigned duties and responsibilities of the agency. May be required to work after 5PM or weekends.
Supervised by: SSVF Program Manager
Salary: $17.30 to $19.23 per hour plus excellent benefits.
Schedule: 40 Hours per week, Flexible, Monday – Friday 9:00 am to 5:00 pm tentative
