Wallingford Emergency Shelter Case Manager

SEASONAL: December 3, 2018 to March 31, 2019

To provide case management services to 15 homeless adults (men and women) in the greater Wallingford area who may have struggled with substance abuse, mental illness, and other issues related to their homelessness.


Duties and Responsibilities
Provide linkages for residents who are in need of social, mental health, educational, vocational, recreational, spiritual, and or substance abuse services. Coordinate and collaborate with other agencies. Attend weekly and monthly Columbus House, Program and network meetings. Monitor all cases assigned and complete all of the documentation/charting required by the Program. Other duties as assigned.


Minimum Job Requirements
Valid Driver’s License, Associates Degree or two years work experience in a social service agency as a Case Manager, CDAC or CDAC eligible preferred, computer literate and physically able to move independently through the community.


Knowledge, Skills, and Abilities Required
Able to learn HMIS database software program; have knowledge of the greater Wallingford substance abuse, mental health and social services network of providers; empathetic engagement skills; person-centered planning; cultural competencies; life training and group skills. Must be reliable, punctual and able to maintain client confidentiality while respecting proper relationship boundaries; must be ready and willing to accept direction and guidance from supervisors.


Supervised by: Middlesex Emergency Services Manager
Salary Range: $17.30 to $19.23 per hour, no benefits
Schedule: 40 hours per week, Monday through Friday 12:00 p.m. to 8:00 pm. Schedule may be modified during snowstorms or when the State of Connecticut’s Governor activates the Cold Weather Protocol.
