DOC RHAP Rapid Re-Housing Case Manager

Purpose

Under the direction of the Rapid Re-Housing Manager, the DOC RHAP (Department of Correction Re-entry Housing Assistance Program) Rapid Re-Housing Case Manager assists people on discharging from the CT DOC by assisting in locating housing and coordinating a structured move from incarceration to community living. The DOC RHAP RRH Case Manager provides case management services to help individuals in finding, securing and maintaining housing and will provide linkages to educational, employment, vocational, spiritual and recreational opportunities in addition to mental health, substance abuse and medical treatment.

 

Qualifications, Knowledge, Skills and Abilities

Bachelor’s Degree in Human Services or relevant field with 2 years’ relevant experience or an Associate’s degree with three years human services experience. Knowledge of Fair Housing Laws and supportive housing principles and procedures; knowledge of community support systems and resources. Knowledge of leasing procedures, housing quality standards and inspection methods. Knowledge of Department of Correction Re-Entry procedures. Excellent organizational and time management skills with a deadline-oriented mindset. Strong written and oral communication skills employing the English language. Proficient is Microsoft Office and other common desk top applications. Able to work with diverse populations. Sound judgment and decision-making ability. Crisis intervention skills, boundaries, confidentiality, universal precautions, harm reduction, strong empathetic skills. Possessed of a strong desire to promote equal access and opportunity for individuals leaving incarceration. Familiar with mental health, substance abuse and dual diagnosis issues and dynamics. Ability to collaborate successfully with others. Able to meet all documentation requirements. Must be able to multitask. Valid drivers’ license with a good driving history.

 

Duties and Responsibilities

Conduct intake/enrollment, treatment planning, ongoing clinical case management (assessment, resource linkage, crisis intervention, case planning, consultation, psychoeducational) and financial skill building. Conduct shelter diversion/housing solutions conversations with client’s pre-release. Connect clients to needed services and supports to help them stabilize in housing and thrive; including but not limited to employment opportunities, recovery assistance programs, mental health services, basic needs, benefits and natural supports. Housing location assistance and education, move in assistance, landlord liaising, regular household budgeting, credit repair work as applicable, twice monthly visits to each participant’s home once housed to continue to work towards housing stabilization goals. Must be able to drive with own vehicle and conduct themselves professionally in the community. Provide mediation between family members and client. Attend regularly scheduled supervision, team or staff meetings as requested. Maintain strict confidentiality concerning both program applicants and accepted clients while carrying out all duties. Other related duties as assigned.

Salary: $17.30 to $19.23 per hour plus excellent benefits.

Schedule: 40 Hours per week, Flexible, Monday – Friday 9:00 am to 5:00 pm with flexibility
