Health Navigator
(Position located in Middletown)

Purpose
Under the direction of the Manager of Middlesex Emergency Services, the Health Navigator, Outreach and Engagement acts as the support hub for all aspects of the client’s movement through the healthcare system, particularly focused on providing education and encouragement for Covid-19 prevention and vaccination among the unsheltered population in the MMW CAN. The Navigator’s role is to promote smooth and timely continuity of care to the point of resolution. The Navigator will provide connections to mental health providers and to advance the overall well-being of our unsheltered population. The Navigator serves as an advocate for patients seeking and receiving care in multiple health care settings and interprets the needs and concerns of the clients for other staff members and provides linkage/access to care, patient registration, appointment scheduling, collaboration with the treatment community arranging for transportation, follow-up, quality management activities, data collection and Covid-19 education.


Qualifications, Knowledge, Skills and Abilities
A minimum of two years’ experience working in human services or related medical fields. Excellent organizational and time management skills with a deadline-oriented mindset. Proficient is Microsoft Office and other common desk top applications. Able to effectively use the local network of substance abuse, mental health, and social services providers. Familiar with criminal justice and able to engage, manage, motivate, and support. Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics. Complete knowledge base of all aspects of the delivery of person-centered case management. Able to meet all documentation requirements. Knowledgeable about crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, trauma informed care and motivational interviewing. Valid drivers’ license with a good driving history.


Duties and Responsibilities
Provide education on prevention of Covid-19 through testing, vaccinations, and other preventative measures including washing hands and wearing masks correctly. Eliminate barriers to timely screening, diagnosis, treatment, and supportive care for unsheltered homeless individuals in the MMW CAN. Schedule and/or provide transportation to appointments for clients as appropriate. Coordinate and collaborate with Outreach and Engagement team as well as other agencies and social service providers. Ensure that clients understand the importance of routine screening and follow-up care. Plan, coordinate, implement and evaluate patients’ understanding of provided health education. Monitor all cases assigned and complete all documentation/charting required. Register clients and schedule appointments in order to facilitate accessible linkage to primary care and ancillary services. Follow-up with clients to determine if the appointment was kept and if there are any additional health education needs. Maintain and update weekly charting and movement reports. Other related duties as assigned


Supervised by: Manager of Middlesex Emergency Services
Salary: $15.00 plus excellent benefits
Schedule: 40 Hours per week, Flexible, Monday – Friday 9:00 am to 5:00 pm
