HOPWA /Scattered Sites Housing Case Manager

Objective
To provide case management services to formerly homeless adults, now living in long term supportive housing apartments in the Middlesex County area, in order to assist individuals in maintaining housing and improving their quality of life.


Duties and Responsibilities
Improve the resident’s quality of life by, among other things: providing linkages for those who are in need of social, mental health, educational, vocational, recreational, spiritual, HIV, and/or substance abuse services. Provide transportation to appointments for residents; coordinate and collaborate with other agencies. Develop mutually reinforcing relationships with landlords and assist residents with finding housing as needed. Attend weekly and monthly agency, program and network meetings. Monitor all cases assigned and complete all documentation and charting required by Columbus House, HOPWA,HUD, DMHAS, Shelter Plus Care. Other related duties as assigned.


Qualifications
Bachelor Degree preferred or Associates Degree with 2 – 4 years relevant work experience, computer literate and physically able to move through the community. Must have knowledge of the Middlesex County area substance abuse, HIV, mental health and social services network of providers. Familiar with mental health, substance abuse, HIV and dual diagnosis issues and group dynamics. Must be reliable, punctual and able to maintain client confidentiality while respecting proper relationship boundaries; professional, ability to work independently and as part of a team, ability to foster teamwork, problem solving capabilities. Must be able to work between multiple housing programs. Strong verbal and written communication skills. Must be able to engage, motivate and support. Valid CT Driver’s License with a clean driving record. Time management skills and computer literate with a deadline-oriented mindset. Ability to meet all documentation requirements; complete knowledge base of all aspects of the delivery of person-centered case management. Ability to assess strengths, barriers, and needs to assist tenant in identifying personal goals, values and priorities, ability to advocate for tenant as well as support self-advocacy. Flexible, crisis intervention skills, confidentiality, universal precautions, harm reduction, strong empathetic skills.


Supervised by: Permanent Supportive Housing Manager
Salary: $17.30 to $19.23 per hour plus excellent benefits
Schedule: Monday – Friday 9:00 a.m. to 5:00 p.m. with some flexibility.
