Purpose
To support and link individuals experiencing homelessness to housing, educational, employment, vocational, spiritual, recreational, medical, and treatment resources; to reduce the numbers of men and women in Meriden/Middlesex County/Wallingford who struggle with mental illness and substance abuse which has or can lead them to homelessness. Qualifications, Knowledge, Skills and Abilities BA degree required, Masters preferred or four years plus experience working as a case manager in outreach/substance abuse position. The experienced individual will work with people who are homeless, and who have a history of mental health issues and substance abuse and who are currently disengaged from services. Knowledgeable of outreach activities, engagement skills, basic entitlements, housing, state and community resources, and mental health issues. Must have organizational and network skills to create linkages to medical, mental health, substance abuse treatment, employment, and educational opportunities; sensitivity to working with people with various cultural backgrounds and must have knowledge of Trauma-Informed and Gender-Specific care services. Computer literate with the ability to learn new software programs required. Must have a valid driver’s license and a good driving record. Harm Reduction skills and PTSD knowledge a plus. Must be comfortable engaging in street outreach and providing a full range of case management services to individuals in the community, i.e., in soup kitchens, shelters, behavioral health providers, and other community partners. Bi-lingual English/Spanish preferred.

 

Responsibilities
Outreach to individuals with mental health and substance abuse who are homeless or at-risk of homelessness and are disengaged from services. Collaborate with other outreach staff, shelter case managers, and other community providers. Enroll a minimum of 20 clients each grant year. Enter all client data into the Homeless Management Information System (HMIS) in timely fashion. Documentation must be accurate and completed in a timely manner, including, but not limited to, First Contact Forms, Mental Health and Substance Use screening tools, Person Centered Service Plans, and Referral Forms. Provide case management services to connect individuals to mainstream benefits and services, including emergency shelter, behavioral health services, substance use treatment, vocational and employment, housing, and other community resources. Attend network and staff meetings, regular bi-weekly supervision session with supervisor. The position will also require staff to collaborate with the Coordinated Access Network Entry and Exit process in Meriden/Middlesex/Wallingford region. Staff will be required to outreach during early morning hours or late evening hours throughout the year. Other related duties as assigned.

Salary Range: $17.30 to $19.23 per hour plus excellent benefits

Schedule: Monday – Friday (20 hours; flexible schedule, some evenings)
