Rapid Re-housing Housing Specialist

Under the direction of the Rapid Re-housing Manager, the Rapid Re-housing Housing Specialist Works with the housing department to identify and recruit landlords conduct inspections and process paperwork to prepare units for occupancy. This individual will also maintain a caseload and provide on-going support to eligible applicants with case management and connections to area services including employment, behavioral health, physical health and living supports.
Qualifications, Knowledge, Skills and Abilities Required
Bachelor’s Degree in Human Services required with a minimum of two years’ experience working in human services or related fields. Master’s degree desired. Valid drivers’ license with a good driving history. Excellent organizational and time management skills with a deadline- oriented mindset. Strong written and oral communication skills employing the English language.
Proficient is Microsoft Office and other common desk top applications. Able to effectively use the local network of substance abuse, mental health, and social services providers. Familiar with criminal justice and able to engage, manage, motivate, and support. Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics. Complete knowledge base of all aspects of the delivery of person-centered case management. Able to meet all documentation requirements. Knowledgeable of crisis intervention, boundaries, confidentiality, universal precautions, harm reduction. Knowledge in housing first, housing law and tenant rights. Familiar with landlords in the greater New Haven area.
Duties and Responsibilities
Provide linkages for clients who are in need of social, mental health, educational, vocational, recreational, spiritual, and or substance abuse services. Ensure that those who need mainstream benefit programs (e.g., SSI/SSDI, TANF, SNAP, etc.) are referred to the SOAR worker for eligibility screening. Continuously prepare the client for transfer of care to permanent housing and recovery support caregivers. Coordinate and collaborate with other agencies and social service providers. Develop mutually reinforcing relationships with landlords and assist residents with finding housing as needed. Attend weekly and monthly agency, program and network meetings. Monitor all cases assigned and complete all documentation/charting as required. Create strength-based person-centered service plans Other related duties as assigned.
Supervised by: Rapid Rehousing Manager
Salary Range: $17.30 to $19.23 per hour plus excellent benefits
Schedule: Monday thru Friday 9:00 a.m. to 5:00 p.m., flexibility required.
