Supportive Services for Veteran Families (SSVF) Health Care Navigator
(Position located in New Haven)

Purpose
Under the direction of the New Haven SSVF Program Manager, the SSVF Health Care Navigator will provide services that include connecting veterans to the VA health care benefits or community health care services where veterans are not eligible for VA care. The SSVF Health Care Navigators will also provide case management and care coordination, health education, interdisciplinary collaboration, and administrative duties. Health Care Navigators will work closely with the veteran’s primary care provider and the veteran’s assigned multidisciplinary treatment team.


Qualifications, Knowledge, Skills and Abilities
Master’s Degree in Social Worked required with a minimum of one year experience in health care or social services area of practice. Excellent organizational and time management skills with a deadline-oriented mindset. Strong written and oral communication skills employing the English language. Proficient in Microsoft Office and other common desktop applications. Able to effectively use the local network of substance abuse, mental health, and social services providers. Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics. A complete knowledge base of all aspects of the delivery of person-centered case management. Able to meet all documentation requirements. Knowledgeable about crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, trauma-informed care and motivational interviewing. Valid drivers’ license with a good driving history.


Duties and Responsibilities
Conducts non-clinical assessment of the veteran in collaboration with the interdisciplinary treatment team and family members. Works with the veteran to assist them in communicating their preference in care and personal health-related goals to facilitate shared decision making of the veteran’s care. Provides resources for education and support for veterans and families and help identify appropriate and credible resources that are tailored to the needs and desires to the veteran. Participate in the development of the veteran’s care plan, with an emphasis on community services, outreach, and referrals. Provide linkages for clients who require social, mental health, educational, vocational, recreational, spiritual, and or substance abuse services. Coordinate and collaborate with other agencies and social service providers. Attend weekly and monthly agency, program, and network meetings. Enter and maintain all client charts accurately and timely into the Homeless Management Information System (Case Worthy). Assist or transport veterans to health care appointments. Other related duties as assigned. Strong empathetic skills. Able to assess strengths, barriers, and needs to assist clients in identifying personal goals, values, and priorities. Ability to lead Inter-disciplinary case conference


Supervised by: SSVF Program Manager
Salary: Negotiable depending on education and experience; excellent benefits.
Schedule: 40 Hours per week, Flexible, Monday – Friday 9:00 am to 5:00 pm
