Supportive Services for Veteran Families (SSVF) SOAR Benefits Specialist

Purpose

Under the direction of the Director of Middlesex and Veteran Services, the SSVF SSI/SSDI Outreach, Access, and Recovery (SOAR) Benefits Specialist will assist homeless and at-risk veterans and anyone in their household who have mental illness or a co-occurring substance use disorders, or other medical impairments apply for Social Security Disability Benefits in support of the Supportive Services for Veteran Family’s program.

Qualifications, Knowledge, Skills and Abilities

Bachelor’s Degree in Human Services or related field. Minimum three years’ relevant experience working directly with individuals and families experiencing homelessness who have a mental illness or are dually diagnosed. Successful completion of the SOAR on-line training course through SSA within 30 days of hire. Complete interviews with consumers to gather information to complete SSI/SSDI Applications. Gather medical records and other information needed to complete SSI/SSDI Applications. Good writing skills and the ability to analyze extensive data and create written reports with accuracy Complete a SOAR Medical Summary Report for consumers applications. Accompany clients to appointments to the Social Security Administration. Coordinate visits to medical doctors, psychiatrist, and other specialist to obtain evidence for case. Coordinate case management services with partners and help with providing case management services to clients when needed. Working knowledge of behavioral health service models, including the recovery model. Familiarity with Social Security Administration processes for applying for federal benefits. Excellent organizational and time management skills with a deadline oriented mindset. Proficient is Microsoft Office and other common desk top applications. Familiar with mental health, substance abuse and dual diagnosis issues and dynamics. Ability to collaborate successfully with others. Able to meet all documentation requirements. Knowledgeable of crisis intervention, boundaries, confidentiality, universal precautions, harm reduction and motivational interviewing. Valid drivers’ license with a good driving history.

Duties and Responsibilities

Collaborate with SSVF service team. Coordinate the SOAR application submission process for all SSVF program sites through a comprehensive review of each application for accuracy and completeness. Act as liaison to Medical Records and other hospital departments; Disability Determination Services, Social Security district office and other relevant services when necessary to secure provision of documents. Direct the appeals process as necessary. Attend weekly and monthly agency, program and network meetings. Monitor all cases assigned and complete all documentation/charting required by Columbus House and SSVF requirements. Other related duties as assigned

Supervised by: Director of Middlesex and Veteran Services

Salary: $17.30 to $19.23 per hour plus excellent benefits.

Schedule: 40 Hours per week, Flexible, Monday – Friday 9:00 am to 5:00 pm tentative
